PRCERTIFICATION: All inpatient and outpatient mental health and substance abuse services must be precertified through CORPHEALTH. The phone number to call is 1-800-777-6330. Failure to obtain precertification for mental health or substance abuse services will result in benefits paid out-of-network level (refer to the schedule of benefits). It is your responsibility to contact CORPHEALTH to identify providers in their network.
