What is a Skilled Nursing Facility?
A skilled nursing facility offers a place for those who need short-term rehabilitation following a hospital stay for illness or injury because of health issues or disabilities. All patients are under the care of physicians, who supervise each patient's progress, and are responsible for each patient's overall plan of care. Our goal is to restore each patient as quickly as possible to their highest level of independence and functioning.

How do I get Skilled Nursing Care?
The decision to move the patient to a skilled nursing care facility is initiated by the physician and the case manager. Skilled nursing care is a choice for you, your family and your physician. To facilitate the process at any St. Joseph critical access hospital, family members should work with case managers and physicians to contact the facility of their choice. After the facility is contacted, medical and financial assessments are performed to determine the patient's eligibility.
Is Skilled Nursing covered by Medicare, Medicaid and private insurance?
Most medical insurance coverage follows Medicare guidelines. Private insurance and Medicare pay for Skilled Nursing for certain time periods following a hospital stay. A representative from our case management team or patient financial services team can advise you on your eligibility for services.
Reasons for Skilled Nursing
We are equipped to care for individuals needing 24-hour nursing care, post-operative rehabilitation, as well as chronically ill individuals Some reasons for care include:
- Wound care and dressing changes after a major surgery
- Administering and monitoring IV antibiotics
- Nutritional therapy following feeding tube placement
- Physical therapy to correct strength and balance problems that have made it difficult for a patient to walk, use the toilet, or get up and down from furniture.
- Occupational therapy to help relearn independent self-care such as dressing, grooming and eating
- Respiratory therapy to give breathing treatments or physiotherapy
- Rehabilitation and recovery from surgery
