Advance Medical Directives

(Commonly called a “Living Will”)

What are they?
They are legal documents that provide you a way to direct your future medical care.

Why are they important?

  • You have the right to accept, or refuse, medical care and to participate in the decisions regarding your care.
  • These documents provide you with a reliable way to make your wishes known.
  • These documents will be a great assistance to your doctors and to your loved ones so that they can follow your wishes regarding future medical treatment.

    What Advance Directive Documents are available?

    (You will need Adobe Reader to download these documents.)

  • Directive to Physician and Family or Surrogates - This is the document that was formerly called a "Living Will." The Texas law has changed the title. In this document, you make choices about your end-of-life care.

  • Medical Power of Attorney - In this document, you specify a person or persons to make medical decisions for you if you cannot make them yourself. This is different from a Durable Power of Attorney, which specifies a person to take care of financial and other matters.

  • Out of Hospital DNR - This form notifies emergency personnel that you do not want to be resuscitated or placed on life support. "DNR" stands for "Do Not Resuscitate."

  • Mental Health Treatment Declaration - This document records your decisions about any future mental health treatment you may receive.

    What Do I Do Next?

  • Download the forms provided on this website by clicking on the links immediately above.
  • Complete the forms according to your wishes and sign them in the presence of two witnesses.
  • Place the original(s) in a safe place.
  • Make copies and give one to your primary care doctor. Bring a copy with you when you are admitted to any hospital. Give a copy to any person whom you name as your agent in the Medical Power of Attorney.
  • "Just Do It!" (It is easy to put this off and never get around to it. Don’t wait until it is too late!)
